Welcome to Centers for Family Medicine, a Medical Corporation. We have two convenient locations in Los Alamitos and Cypress. Our qualified physicians and staff make a special effort to make you feel comforted. The following is a list of some of the many services we provide at our office:

What to bring to your appointment:

- Please complete updated registration and patient information forms. We need to have your information updated on an annual basis.
- Bring names, addresses, and phone and fax numbers of your former treating physicians. This will help us request your records, if needed. A form to request your old records is available. A copy of our privacy policy is available for your review.
- Bring all medications that you take, including non-prescription pills and vitamin and nutritional supplements. Bring all of these in their original bottles. You should also make a list of your medications and carry that with you
- Bring your immunization record, preferably the standard yellow card printed by the state health department. This is especially important for our pediatric and adolescent patients.
- We encourage you to complete and bring a copy of your Advance Healthcare Directive.
- If you are under 18 and a parent or guardian will not accompany you to your appointment, please complete Consent to Evaluation and Treatment Form.
- Bring a state-issued identification.
- Bring your medical insurance card.
After Hours Care:

- A physician is on-call 24 hours per day.
- Our offices make every effort to accomodate same-day evaluations for urgent medical problems.
